What do families need to know financially, logistically, and emotionally when caring for someone with a chronic illness?

Join us for a discussion and Q&A with Jordana Kozupsky Bel, AGNP-BC, ACHPN, a geriatric and palliative care nurse practitioner at Phelps Hospital. She will address questions on planning for the future, end-of-life care, advanced directives, and more. Please note: Jordana will not offer specific medical advice.

Free and open to all, but registration is required! Sign up now on Zoom.

Participants may also submit questions during the registration process, or by emailing gglazer@wlsmail.org. All questions will be read anonymously.

This program is a collaboration between Croton Free Library, Ossining Public Library, Putnam Valley Library, and Warner Library.
